How to Prepare for Your DOT Physical

Once you book your DOT exam, we’ll send you the necessary documents. You can complete the paperwork prior to your visit with us, or fill it out at the clinic. 


If any of the items below apply to you, we recommend bringing them to your appointment: 

  • A list of your medications and their dosage
  • A list of doctors you work with, including their name, phone, email, and address
  • If diabetic, your blood sugar reading
  • If you have sleep apnea, a record from your CPAP machine over the last 90 days
  • Your glasses or contacts and a current prescription
  • Your hearing aids
  • The results of any recent health tests that pertain to illnesses, injuries, or conditions you have

 

We will also need letters from your doctor(s) saying you are safe to drive if any of the following apply to you:

  • If you suffer from heart issues
  • If you’ve lost the use of an arm or leg
  • If you take medications that cause sleepiness or are considered a controlled substance
  • If you’ve had a stroke
  • If you have or had a brain tumor
  • If you suffer from seizures or bleeding in the brain

 

Some conditions may disqualify you from receiving your DOT card, including:

  • Unstable heart condition
  • High blood pressure
  • Vision impairment
  • Hearing impairment
  • Seizure disorders or epilepsy
  • Inner ear disorder
  • Diabetes
  • Marijuana use
  • Illegal substance abuse
  • Respiratory conditions
